A Case of Chronic Back Pain.

چکیده

B is a common symptom in middle-aged people associated with significant morbidity and loss of work. While acute back pain (lasting for few days to few weeks) is due to mechanical cause, aetiology is difficult to determine in chronic backache patients (more than three months) and tend to be progressive. We present a case of a middle aged lady with chronic backache, who remained undiagnosed for a decade. A 39yr old female, residing at RajapurMaharashtra, presented with complaint of low backache and bilateral lower limb pain since 10 years followed by inability to walk since 2 months. There was worsening of pre-existing back pain and inability to move both lower limbs since 8 days. She was bedridden for one month. There was no loss of sensation, t ingl ing or numbness or bowel or bladder involvement . No history of trauma, fever, weight loss, malaise, early morning stiffness, uveitis, psoriasis, heel pain, family history of back pain or joint pain. She was taking treatment from orthopaedic surgeon for the back and limb pain and was treated with analgesics, calcium and vitamin D supplements and traction, but there was no improvement and pain progressively increased associated with corresponding decrease in lower limb movement. She had history of depression and mood changes for that she was started on anti depressant treatment with selective serotonin reuptake inhibitor (SSRI) drugs 2 years back. She had regular menstrual cycles. There was history of weight loss of 10 kg over last six months. On examination: patient was afebrile with pulse of 88/min, regular; BP-108/70 mm Hg.
